I am pakker vaishnavi a final year mbbs student from India.

I'd like to share some of the interesting cases that helped me to learn more about medicine.

Case 1 

A 65 old male with shortness of breath ,facial puffiness and pedal edema.

My first case in the medicine ward was a regular dialysis, Sitting in the post-hemodialysis ward, I observed a middle-aged man, approximately 65 years old, accompanied by his son. The patient appeared unwell, with a noticeable swelling of his legs , exhibiting significant pedal edema.He also had the puffiness around his eyes and cheeks further emphasized the severity of his condition. It was evident that he was experiencing discomfort, as he struggled to breath. Even simple activities, such as talking or walking to the washroom, seemed to exacerbate his shortness of breath.The laboratory findings revealed elevated levels of serum creatinine, urea, and alkaline phosphatase, coupled with low protein levels. These abnormalities hinted at a severe renal dysfunction and disruption in the body's normal metabolic processes.I learned that the elevated serum creatinine and urea indicated a significant impairment in his kidney function, resulting in the accumulation of waste products and toxins in his body. This, in turn, contributed to the development of pedal edema, facial puffiness, and decreased urine output.He have been undergoing dialysis since 8 months intially dialysis was done weekly once now dialysis is done for every two days .


 In The next case I would like to explain about viral pyrexia 

A 40 year old female patient presented with chief complaints of low grade fever since 4 days and nausea since 3 days ,generalized weakness and loose stools since a day.

On history taking we are able to determine the cause of viral pyrexia is unhygienic food intake .

We explained cleanliness measures and the importance of it in prevention of disease . And the USG reports revealed tha she has fatty liver grade 2 which could be due to excess calories intake .

It solidified my commitment to provide the best care possible and find answers for patients like them.

Case 3

A 42 years old male presented with chief complaints of fever , vomiting , burning micturation since 3 days .

On history taking the patient was found to be diabetic since 2 months which was a incidentally found .And he also complained about decreased urine out put and increased frequency due to osmotic diuresis due to which he suffer with inadequate sleep. He is a known case of hypertension since 4 years.

I realized the significance of formulating a comprehensive treatment plan . It was essential to address his problem, enhance his quality of life.
